What Is Adult Foster Care (AFC)?

AFC Services is a MassHealth program that are ordered by a Physician and delivered to a Member in a qualified home setting by a Qualified Caregiver. The Member will receive quality care at-home by a multidisciplinary team of Register Nurse services and oversight, AFC Provider Management, and qualified AFC Caregiver. Services include assistance with activities of daily living (ADLs), Instrumental Activities of Daily Living (IADLs), and other personal care as needed.

Who Is An AFC Member?

An eligible AFC Member is a 16 years of age or older with medical or behavioral conditions that prevent him/her from fully taken care of self on a daily basis. Individuals who need assistance or support to perform the following task independently, such as: eating, bathing, dressing, grooming, toileting, transferring. household-management tasks, laundry, shopping, housekeeping, meal preparation and cleanup, transportation (accompanying the member to medical providers and other appointments), care and maintenance of wheelchairs and adaptive devices, medication management or care for other basic daily activities qualifies to become an AFC Member.

Ongoing Support:

Enrich Wellness Adult Foster Care AFC Care Managers and RN’s are in regular contact with the Member and Caregiver, visiting the home a minimum of one time per month. The staff provides on-going training to ensure that the needs of the Member are being met.
